Summary: Sonoma County Indian Health Project, Inc. (SCIHP) was established in 1971 by a group of leaders from the Indian communities of Sonoma County. Their goal was to provide health care for all Indians of Sonoma County and to provide services in a manner which is sensitive to the culture and traditions of the local Indian Tribes. Over the years, SCIHP has steadily progressed from a small organization consisting of a single dental chair and an exam table to the multimillion-dollar comprehensive health care facility it is today. Even in these turbulent times of health care and diminishing resources, we continue to grow. Because SCIHP is funded by State, County and private funds, we also provide care to non-Indians depending on the funding source. SCIHP has now expanded care to non-Indians with Medi-Cal on a limited basis. Cloverdale, Dry Creek, Graton, Lytton, Manchester Point Area and Stewarts Point Rancherias have designated SCIHP as a Tribal Organization, under P.L. 93-638. The Mishewal Wappo tribe is also one of the tribes that support SCIHP. SCIHP has 501(c)(3) status and is incorporated by the State of California as a non-profit organization.

Duties: Job Purpose: The RN Case Manager is a member of the clinical Care Team with the primary focus of assisting providers and patients in the areas of nursing case management. The RN Case Manager will work as part of the clinic nursing team to provide a full range of nursing services including floor nursing, patient triage, phone advice, case management, Care Team coordination, and patient education. Qualifications: Qualifications: • Graduate of accredited school of Nursing. • Current unrestricted license to practice nursing in the state of California. • One (1) to Two (2) years of clinical nursing experience preferred. • Work closely with the Primary Care Provider and clinical Care Team in providing nurse case management. • Participate in patient and staff education. • Provide nurse visits for designated conditions per clinical protocol. • Coordinate Care Team support for patients with new health management goals. • Assist with discharge planning and home care referrals. • Oversee Care Team Medical Assistant administration of immunizations and medication, both injection and oral within the office visit. • Always maintain patient confidentiality; abides by HIPPA and other regulations put forth by various regulatory agencies. • Ability to work well within a multidisciplinary team. • Able to operate EKG, microscope, centrifuge, glucometer, nebulizer, Cholestech, HgA1c and Clinitek machines. • Must pass a pre-employment drug screen, physical and background investigation. Special Qualifications: Must be sensitive to the needs of the Native American Community, which includes their culture, traditions, and background.
