Providing comprehensive and continuing health and medical care services in full scope Family Medicine practice. The health care provided requires the recognition of diseases and injuries, which require referral including those which are difficult to discover or identify.
Duties: FAMILY MEDICINE PHYSICIAN: ESSENTIAL FUNCTIONS 1. 1. Interviews and examines patients, reviewed past medical history, and requests and/or performs diagnostic tests and examinations deemed necessary to obtain all possible information related to each case. Makes preliminary diagnosis, directs, prescribes or provides treatment, or arranges for specialized care or patient referral as required. Bases decisions upon information obtained, professional medical knowledge and skills, and prescribed policies and procedures. Make notes of observations to be incorporated into clinical records. Provides emergency care as required. Provides definitive management in all medical categories for cases that do not require referral. Gives total patient care including prevention, health maintenance, early diagnosis, treatment and follow-up services to patients under his/her care. 2. Conducts outpatient clinics making observation appropriate to the outpatient; underlying disease and treatment plan, including current symptoms. Reviews the patient's medications, diet, therapy course, adherence to management plan and patient observations. Modifies regimen as indicated. Instructs and counsels patients and family members as required. Performs or assists other physicians in the performance of minor surgery. 3. Provides prenatal care and counseling, and may deliver infants, obtaining consultation as deemed necessary, and provides or arranges for postpartum care. 4. Refers patients to appropriate contracted medical or other government facilities, providing full clinical information for care and diagnostic procedures that cannot be adequately provided here. Coordinates and integrates information obtained into the ongoing Health Care Program at TCRHCC and assures the provision of timely follow-up care as required. 5. Assures preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Dictates clinical reports and correspondence concerning patients under his/her care to physicians, hospitals and other individuals and facilities. 6. Works closely with allied health professionals (i.e., Community Health Nurse, Social Workers, Health Educator, etc.) to utilize and coordinate the services of other health care professionals in the management of the patient's medical, psychological and social problems. 7. Trains, teaches and supervises Medical Students and Residents. PHYSICIAN ASSISTANT - NURSE PRACTITIONER: ESSENTIAL FUNCTIONS 1. Provides comprehensive diagnostic, preventative, and therapeutic health care services to patients. Documents visits in the Medical records of patients including updating problem lists. 2. Interviews, assesses, and evaluates patients to determine their physical condition and type of medical services needed, and orders initial diagnostic studies appropriate for the patient's medical condition. 3. Performs a basic physical assessment using techniques of observation, inspection, auscultation, percussion, palpation and diagnostic instruments. Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional, and mental problems. 4. Consults and collaborates with other health care professionals to establish health care plans and to review patient progress and response to therapy, including the use of written referrals to Community Health Nurses, specialty clinics, schools, etc. 5. Adheres to medical regimens to stabilize chronically ill persons and adjusts regimens within established protocols. Makes decisions concerning medical care needs of patients consulting with physicians as appropriate. Works collaboratively with the physician in management of selected complex medical problems. 6. Provides health care and preventive services to healthy individuals including immunizations, guidance in nutrition, common illnesses, accidents, child growth and development, substance abuse, exercise, and depression screening. 7. Teaches patients and family members ways to maintain or improve their health status. 8. Carries out treatments and procedures as required, administers medications; observes and documents changes in patient's condition, reactions to medications and treatments. Responsible for prescribing medications including computation of doses. 9. Participates in planning, instituting, evaluating, and revision program plan, assist in determining conditions, resources, and policies essential to delivery of health care services. Participates in a teaching capacity in the development of other health care workers. 10. Participates in performance improvement initiatives which may involve chart reviews, studies relevant to health care and nursing needs of patients. 11. Keeps abreast of the latest medical knowledge via CME as required by respective licensing agency. 12. Handwriting must be legible and must be willing to use an Electronic Health Record. 13. Performs other duties as assigned.
Qualifications: FAMILY MEDICINE PHYSICIAN: NECESSARY QUALIFICATIONS Education: Must have a Medical Degree, MD or DO from an accredited school of medicine. License: • Must possess and maintain a current and unrestricted license to practice as a physician in any State Territory, District of Columbia or the Commonwealth of Puerto Rico • Board Eligible or Board Certified by the American Board of Family Practice or Preventative Medicine. Experience: Completion of three (3) years of accredited residency training in Family Practice or Preventative Medicine. Credentialing: Must become a credentialed member of the TCRHCC Medical Staff PHYSICIAN ASSISTANT: NECESSARY QUALIFICATIONS Education: • Bachelor of Science (BS) degree in Physician Assistant Studies • Current BLS Certification License/Certification: • Must possess and maintain a current and unrestricted license to practice as a physician assistant in any state within the United States, District of Columbia or the Commonwealth of Puerto Rico. • Must possess a certificate from the NCCPA (National Commission for Certified Physician Assistants) Credentialing: • Must be credentialed to serve on the Medical Staff (provide all related information as requested for credentialing process). NURSE PRACTITIONER: NECESSARY QUALIFICATIONS Education: • Bachelor's Degree • Additional requirements will be added to assure development and education needs are met depending on the experience of the incumbent • Must have completed an accredited Nurse Practitioner Program Licensure/Certification: • Must possess a valid, current, full and unrestricted license to practice as a nurse practitioner in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico • Must possess a certificate from the American Association of Nurse Practitioners Credentialing: Must be credentialed and privileged to serve on the Medical Staff Experience: At least 2 years of experience as a nurse practitioner evaluating and treating patients
