BACKGROUND

This transmittal forwards a revision of Part 4, Chapter 3, Section 1 of the Indian Health Manual.  This section provided Indian Health Service policy, procedures and scope of reporting on outpatient medical services provided to Indian and Alaska Native people or others, and henceforth, will be know as "Ambulatory Patient Care Reporting".

This system utilizes a computer to provide the needed reports for administration and planning at all levels, except communicable disease reporting to State Departments of Health.  The forms to be used in connection with these procedures are:

HSM-406 (Rev. 7-73) - "Ambulatory Patient Care Report"
HSM-405 - Ambulatory Patient Care Master Form"
HSM-407 (Rev. 7-73) - "Grouped Services Report"
State Department of Health Weekly and Individual Communicable Disease Report Forms

The State Department of Health weekly and individual communicable disease report forms shall be obtained by each facility from their respective State Health Department offices.
