Search, Sort and Download tips  click on icon to open help Search Code (autotextsuggest field), Provider (empty field), Status (drop down list), Wl Flg (drop down list), Prm Prov Flg (drop down list) and Definition (empty field) columns only or any combination of these columns.

Autotextsuggest pulls values from the field in the database that begin with the string (not case sensitive) entered by the user and displays them as drop down list options. These values populate as the user types. Drop down list searches for results that are an exact match with the value selected by the user. From drop down list, select value 'Empty' to select results where the field is not populated. Empty field searches for results that begin with, include or end with the string (not case sensitive) entered by the user.

To Sort the columns in a table click the column header(s) that is bolded and underlined. Columns will sort ascending (any unpopulated will display at the top) or descending.

To Download an entire table, under 'Download This Table', select the Download All, to download the filtered table, under 'Download This Table', select the Download Current View. click on icon to close help

64  NEPHROLOGIST  Active  An individual who specializes with the branch of medicine dealing with the kidney. 
67  CLINICAL PHARMACY SPECIALIST  Active  An individual who is certified by the IHS National Pharmacy Credentialing Committee as a Clinical Pharmacy Specialist (CPS), for those patient care visits falling under the CPS scope of practice. This code also includes patient care visits by pharmacists who are locally privileged by the Medical Staff to perform this level of care, but are not yet certified nationally by the Credentialing Committee. 
68  EMERGENCY ROOM PHYSICIAN  Active  An individual who practices emergency medicine in a hospital emergency department. An ER physician can be board certified after successfully completing a residency in emergency medicine and has passed the board exam in emergency medicine. Emergency medicine focuses on diagnosis and treatment of acute illnesses and injuries that require immediate care. 
69  CHIROPRACTOR  Active  An individual who employs the doctrine and dogma of chiropractic. The science and art of restoring or maintaining health, based on the theory that disease is caused by interference with nerve function, and employing manipulation of the body joints, especially of the spine, in seeking to restore normal nerve function.

71  INTERNAL MEDICINE  Active  Physicians who are skilled in disease prevention and in managing complex disorders of the body. 
72  OB/GYN  Active  A specialist in this field; an obstetrician-gynecologist. Obstetrician is a doctor who specializes in pregnancy and childbirth. Gynecologist is a doctor who specializes in conditions that affect a woman’s reproductive organs. 
73  ORTHOPEDIST  Active  A specialist who prevents, cures, or remedies deformities of the skeletal system. 
74  OTOLARYNGOL  Active  A physician specialist who is an expert in the medical and surgical treatment of disorders of the larynx and ears.

76  RADIOLOGIST  Active  An individual who is a graduate of a radiologic technology program accredited by an accreditation body recognized by the U.S. Department of Education, is currently certified as a radiologists, meets any current legal requirements of licensure or registration or has the documented equivalent in education, training, and experience, and is currently competent in the field. 
77  SURGEON  Active  A physician specialist expert in the surgical management of disease; one whose profession or occupation is to cure diseases or injuries of the body by manual operation; one whose occupation is to cure local injuries or disorders (such as wounds, dislocations, tumors, etc.) whether by manual operation or by medication and constitutional treatment.
