Retirement
Retirement is a process, not an event. It is encouraged that you consult with your Regional Commissioned Corp Liaison Office for further information and questions you may have. This page will assist you in only initiating the paperwork to get your retirement process started.
Please be aware that Commissioned Corps requires retirement packets to be submitted to them 120 days prior to your last physical day at the duty station. However, your IHS Regional Commissioned Corps Liasion Office requires you to submit your forms 150 days before your last day at the duty station. This will ensure that your paperwork is completed thoroughly upon their review.
(Please do not sign any of your forms until your Regional Liasion Office has reviewed them.)

(Caution if completed paperwork is not submitted promptly and correctly, it may hinder you from retiring on the retirement/separation date you are requesting. Also, you could be divested of travel and transportation of Household Goods (HHG) and be denied the transfer of, use of, and payment for any unused annual leave.)

(Reminder: Submit these forms to your Regional Liaison's Office for review before signing. To ensure completeness, the Regional Liaison Office will submit your Retirement Packet to PHS Separations at Commissioned Corps Headquarters.)
Per CCI 384.03, An officer who is requesting retirement must also submit a copy of their Retirement Seminar Certificate or other proof that you completed the USPHS Commissioned Corps retirement seminar.

This program is provided to you to disseminate information concerning retirement types, eligibility for retirement, retired pay, and post-retirement benefits and entitlements.

Per CCI 384.03 Mandatory, Voluntary and Involuntary Retirement, an officer with more than 20 years of creditable active service but less than 30 years must request to retire. The request will be made by memorandum and contains a proposed effective retirement date. (Reminder officers may only retire on the 1st of the month, and if requesting lump sum payment, it must be indicated on the Memo.)
Both you and your supervisor must sign the Request to Retire Memo. [WORD - 29 KB] Your Regional Liaison Office will acquire your Area Director or Department Heads signature.
The Memo will need to be provided on either the official IHS Letterhead or your Tribal Organizations Letterhead.
This Memo can be signed (wet or electronic) and scanned.

Part A as the separating/retiring officer, you will complete this section.
Part B will be completed and signed by your supervisor.
Part C will be completed by the Finance/Budget Officer at the Service Unit or Area Finance Office. Your Regional Liaison Office can assist in getting this signature.

Make sure your PHS-1373 is legible, particularly if the information is handwritten and not typed
#7 Last Day at Duty Station
- The last day at the duty station is the last day you will be physically present and working at your duty station. Whether you are terminating or retiring, your last day at your duty station is your last physical day working in the office.
#8 Terminal Leave
- Terminal leave is defined as the Annual Leave dates you take consecutively after your last day at duty station up until your effective retirement date. This includes the weekend day immediately following your last day at duty station and preceding your effective date.
- All requested leave indicated on the PHS-1373 must be entered and approved in eCorps. Submit a copy of your eCorps leave along with your PHS-1373.
- Terminal dates CANNOT be changed.
#9 Separation Physical Examination
- If you elect to take a physical examination, which is highly encouraged. But you don't know the location or date you are being examined. Please put "TBD" in both.
#10 Travel and Transportation
- If you decide to take advantage of your final move, please check the box for Dependents (if applicable) and/or Household Goods, even if you choose not to move or not.
#11 Mode of Travel
- If you decide to take advantage of your final move, please check the box for "Private Conveyance," even if you choose not to move or not.
#12 I elect travel to:
- For retiring officers: if known, specify your home of selection in this section. You have a three (3) years to initiate and complete your move. Travel is coordinated and paid by the agency. Please contact your Regional Liaison Office or visit the Joint Travel Regulations website if you have travel-specific questions.
- If you don't know your home of selection, please input "TBD" in both city and state.
- For terminating officers: specify the name of the city and state to which you request travel and whether it is your home of record or the place from which you were called to active duty, or a location of lesser distance. You will likely move towards the end of your active duty time, but you have 120 days to initiate and complete your move. Travel is coordinated and paid for by the agency. Unless told otherwise. Please contact your Regional Liaison Office or visit the Joint Travel Regulations website if you have travel-specific questions.
This form is essential regarding your retirement pay and Survivor Benefit Plan Election.

Points of emphasis
- When filling out your name in Section I, #1, please type in your Employee ID# in the same box next to your name (see figure 1). It is your old Direct Access number (seven digits). If you don't know it, please contact your Regional Liaison Office.
- Do not check the "Active Duty Only" box in Section III. When you retire, you are going into a new pay system, so the system won't know your financial information to continue. So you will need to fill out all fields #10-13e (see figure 1)

- In Section XI – Certification, when you and your witness sign the form, the dates of your signatures must match. Forms without matching dates will be returned (highlighted in figure 2)
