BACKGROUND
The purpose of this Transmittal Notice (TN) is to rescind Delegation of Authority (DOA), Administrative #41, "Delegation of Authority for Ordering, Printing, and Distributing Indian Health Service Forms."
This DOA is being rescinded because forms are now readily available from other sources. On April 25, 2005, the National Supply Service Center (NSSC) facility was given permission from the Director, IHS to phase out the inventory of forms stored. The Area Offices were notified on August 4, 2005, that the NSSC would terminate its lease with the Chickasaw Tribe for the facility used for stocking and storing IHS forms. The Area Offices then assumed responsibility for obtaining IHS forms for their facilities.
