Summary: To improve the health care of the Indian people of the Central Valley to the highest level, by improving the quality of health care available.

Promote sound nutrition and physical activity practices to improve health and prevent potential health problems through individual, family, and group encounters for patients, staff and Native American communities.
Duties: A. Provide outpatient nutrition assessments, individualized care plans, counseling and education. B. Instruct patients with diabetes on equipment use and guidelines of self-monitoring of blood glucose. C. Communicates and participate effectively with providers, community health coordinators and other support staff and be an active member of the diabetes care team. D. Maintain blood glucose monitoring supplies inventory, order nutrition education materials and prepare nutrition related materials [news articles, handouts, pamphlets, posters and forms], for Native American patients. E. Contribute to the planning, development, coordination and evaluation of the CVIH Diabetes Program Grant as well as the Grant activities for the Native American Community. F. Write/prepare/transmit monthly/annual reports for the Diabetes Audit, program evaluations. Coordinate and Manage the Diabetes Registry in RPMS System. G. Provide nutrition consultation services for Native American Head Start programs. Assist in menu development, evaluation and approval. Provide child screening and assessments. Provide parent nutrition education classes. H. Any other duties assigned to the employee by his or her direct supervisor or CVIH management
Qualifications: 1. Must have a registration with the American Dietetic Association if applicable or maintain American Diabetes Educator Registration. 2. Maintain a current valid California Driver’s License be insurable and have own reliable transportation. 3. Proficient in computer programs. Demonstrate sufficient knowledge in publishing, presentation and spread sheet programs. 4. Requires an awareness and keen appreciation of Indian traditions, customs and socioeconomic needs. 5. Requires tact, courtesy, discretion, resourcefulness, and good judgment in handling functions of a sensitive nature.
