Tséhootsooí Medical Center (TMC) is located in Fort Defiance, AZ, 30 minutes northwest of Gallup, NM and 175 miles west of Albuquerque, NM. In response to community needs and support of leaders in the health care field led to a feasibility and sustainability study. Therein the establishment of TMC as a P.L. 93-638 affiliate in 2010. TMC's satellite clinic located in Sanders, AZ moved to its newest location and became Nahat'á Dziil Health Center, January 2016. Our newest endeavor is a 47,000 square foot Wellness Center located on the New Mexico campus of the AZ/NM state line and scheduled to open FALL/WINTER 2017. Health on the Navajo Reservation has a unique personal interpretation; the optimal level of wellness is distinctive to each patient. Accredited by CMS, TMC provides general outpatient and inpatient healthcare to 16 surrounding communities. TMC also provides same-day surgery, a diagnostic center, a growing network of primary care, ancillary services, Nahat'á Dziil Health Center and a Wellness Center opening Fall/Winter 2017. |
Duties: | Serves as an Advanced Practice Clinician (Nurse Practitioner/Physician Assistant) providing primary care/walk-in care in the Primary Care Clinic. Provides diagnostic, preventative, and therapeutic health services to patients and family members in the Primary Care Department, with the ability to function as an independent practitioner or with appropriate physician supervision. Secures a health and developmental history from the patient or parent and perform a detailed physical exam, records findings and makes critical evaluations. Performs or requests special screening and developmental tests and diagnostic tests and procedure and interprets the results. Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional or mental problems. Makes decisions concerning medical care needs of patients which may involve various treatment modalities, prescribed medications, consultation and referral when appropriate. Provides direct and independent medical care in the presence of illness or disability in order to maintain life, provide comfort, reduce distress and enhance coping ability. Provides surveillance or medical practices applied to stabilize chronically ill persons; adjusts treatments within established standing orders recognizing when to refer the patient to the physician or other health team members. Works collaboratively with physician in management of selected complex medical problems. Participates with other professionals and agencies involved in providing health care and services required to include immunization and disease control, environmental health and health education. Evaluates the medical aspects of treatment plans periodically, recognize when reassessment by a physician or other health professionals is appropriate. Evaluates total health care needs of patients and develops or initiate plans to meet these needs. Provides medical care and preventive services to health individuals, including guidance in nutrition, common illnesses, accidents, child growth and development and child rearing, and immunizations. Maintains knowledge and proficiency in medical practices through continuing education, staff meetings, and workshops, and home study. Teaches patients and family member's ways to maintain or improve their health status. Maintains records and reports on patients, personal activities and other required information for use in program operation, quality assurance and other purposes. Participates in Quality Assurance review processes. Provides full range of emergency services or crisis intervention including lifesaving emergency procedures in order to stabilize a patient sufficiently until a higher level of care is available. Maintains knowledge and proficiency in medical practices through continuing education, staff meetings, and workshops, and home study. Makes home visits when indicated to provide care based on family needs. Performs other duties as assigned. |
Qualifications: | Experience. One (1) year direct work experience in a hospital setting as a Nurse Practitioner or Physician Assistant. Education. Nurse Practitioner. Completion of an Advanced Nurse Practitioner School in any of the following areas: Accredited Family Nurse Practitioner, Accredited Adult Nurse Practitioner, Accredited Adult-Gerontology, Active unrestricted Advanced Nurse Practitioner license in any U.S. State or Territory in any of the following areas: Family Nurse Practitioner, Adult Nurse Practitioner, Adult-Gerontology, National Certification from the American Academy of Nurse Practitioners. Current, valid, unrestricted Drug Enforcement Agency (DEA) License. Must maintain an active, unrestricted Registered Nurse license from any U.S. State or Territory. Or Physician Assistant. Completion of an accredited Physician Assistant Program. Board Certification by the National Commission on Certification of Physician Assistants. Active, unrestricted Physician Assistant License from any U.S. State or Territory. Current, valid, unrestricted U.S. Drug Enforcement Agency (DEA) License. Mandatory Knowledge, Skills, Abilities and Other Qualifications: Must have Knowledge of established concepts, principles and practices related to Nurse Practitioner/Physician Assistant training, with full consideration of mental, emotional and physical factors. Language Skills: This position requires a knowledge of, and sensitivity to, cultural and language differences. Interpersonal Skills: Must have excellent interpersonal skills in handling interactions with hospital staff, other agencies, groups, and patients and families. Technology Skills: Competent in use of Electronic Medical Records. Other Skills: Must be able to work as a team member and develop productive and cooperative working relationships with all members of the health care team. DEA Registration number, Meets the criteria for Credentialing & Privileging of FDIHB Medical Staff Membership of the FDIHB Medical Staff Bylaws. A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. SALARY IS DEPENDENT ON EXPERIENCE. |
