The Diabetes Nurse Case Manager is responsible for collaborating with all Chapa-De medical providers in Auburn and Grass Valley, as well as other staff across the agency, to improve outcomes for our non-Indian patients with diabetes mellitus.
Duties: This is a full time position (40 - 32 hrs per week). Work collaboratively with the Director of Nursing, Diabetes RN Case Manager and the physician liaison for the Diabetes Department. Participate in interdisciplinary meetings to coordinate education and care across the agency. Develop processes to improve our agency's performance on regulatory measures and indicators of patient care and patient outcomes, including AAAHC, GPRA, HEDIS, PCMH level Run reports of patients as needed. Use electronic information systems to document and coordinate patient care. Perform screening and monitoring tests under nursing protocol or direct supervision of all Chapa-De providers. Within the RN scope of practice, independently assess, educate, and advise patients about optimizing their treatment plans, including insulin teaching. Execute group classes. Expand access to billable services, including group visits with a provider. Outreach to community, county, and state agencies. Understand and promote the mission, vision, and values of Chapa-De both in the work place and in the community. Maintain compliance with all state and federal laws and regulations, as they pertain to position including; HIPAA, Scope of Practice, Accreditation Standards, OSHA and Agency P&P. Develop case management based on current AADE best practices. Perform retinopathy exams as needed. Perform other duties as assigned by supervisor.
Qualifications: Bachelors or Masters Degree in Nursing BSN, Certified Diabetic Educator preferred. Current BLS certification Ability to communicate in a compassionate and professional manner with all patients and families. Ability to interact effectively with all members of the health care team. Ability to utilize multiple technologies for gathering data to meet required measures and to evaluate outcomes. OTHER: This position is contingent upon Grant Funding. Indian Preference.
