The purpose of the Pharmacist I / II is to provide comprehensive pharmacy services consistent with the needs of the people and good medical care within the available budget and personnel. Provides direct care/services to neo-natal, pediatric, adolescent, adult, geriatric, and obstetric patients. Determines that appropriate policies and procedures are being followed in pharmacy activities. This position is for an overnight shift (may be assigned to work on day, evening, nights, weekends and Holidays). In addition the Advanced Practice Pharmacist I serves as a clinical pharmacist for direct clinical pharmacy services for inpatient and outpatient departments. The Incumbent is a credentialed provider that provides medication therapy management services for specialized pharmacy-based clinics. In addition the Advanced Practice Pharmacist II is responsible for all specialized pharmacy –based clinics at the assigned branch of TCRHCC. This includes the management and administration of personnel policies and procedures, providing ongoing analysis of clinical services to develop programs commensurate with the highest quality of care, and to direct and manage programs and activities of clinical services in a manner that ensures the mission, vision, and values of TCRHCC. Incumbent provides technical and administrative supervision to pharmacy staff comprised of professional and non-professional employees; Advanced Practice I and Staff Pharmacists, Pharmacy Technicians, Pharmacy Assistants and Pharmacy Billers. Managerial and Supervisory work provided consists of clinical services within the designated branch of pharmacy services. In addition to Pharmacy functions, the incumbent will be the Assistant Office Manager at the designated Clinic.
Duties: Clinical duties: 1. Conducts authorized outpatient pharmacy clinics for examination, diagnosis and treatment of specialized clinic patients. Obtains the relevant health history; follows and adjusts the developed treatment plan; and monitors the patient's outcome to the prescribed treatment plan. 2. As an inpatient pharmacist, attends multi-disciplinary rounds to provide pharmacy-related input for patients' therapeutic plans. Follow up on recommendations made and adjusts as necessary. 3. Responds to Codes Blue and Level 1 traumas, provides support for dose calculation, rates of infusion, routes of administration, and mixing of emergent IV/IM/IO preparations. Plans, reviews and maintains emergency drugs for emergency situations. Document all clinical activities, update problem lists, and update medication allergy information in the medical records. 4. Participates in pandemic and/or disaster drills that TCRHCC has elected to take part in and including responding to actual disaster events. 5. Reviews inpatient and outpatient medical records for safety, appropriateness and effectiveness of drug therapy with regard patient-specific factors (i.e. age, organ function, disease state). Consults with medical staff when concerns arise. Collaborates with nursing to evaluate medication use in all age groups. 6. Provide pharmaceutical services to focus on therapeutic drug monitoring. Analyze medication regimens to ensure the most effective, least toxic, and most economical treatment plan to enforce and enhance the role of Pharmacy as an active member of the health care team based on current clinical knowledge. Advanced Practice Pharmacist I Clinical Duties: 1. All Clinical Duties listed above 1 thru 6 2. Become a credentialed provider that provides medication therapy management services for specialized pharmacy-based clinics. 3. Utilizes the relative health history and performs limited physical assessment related to the specialized pharmacy clinics. Distinguishes between normal and abnormal findings to recognize early stages of serious physical, emotional and mental problems. Medications may be monitored, titrated and/or altered per specialized pharmacy clinic guidelines; therefore, prescriptions may be initiated, canceled, renewed and/or written to change doses. May order and interpret laboratory tests as appropriate for specialized pharmacy clinics and monitoring Inpatient pharmacotherapy. 4. Participate, assess, prioritize, plan and implement interventions for specialized medication therapy management pharmacy clinics in accordance with facility, departmental policies and with the standards of professional pharmacy practice. Advanced Practice Pharmacist II Clinical Duties: (50%) 1. All Clinical Duties previously listed above Managerial/Supervisory Functions: (50%) 1. Participates in providing education to the service population regarding the IHS and the TCRHCC's mission and service availability to the various communities and on an annual basis assist in implementing a program to educate the community about the mission and services available. 2. Formulates clinical service policies and procedures, interpreting pharmacy administrative policies, directives and procedures to employees, patients and pharmacy administration. 3. Implements, coordinates and supervises all clinical service related activities within the designated branch of Pharmacy Services. Fosters the interest of the staff in clinical service activities and monitors the established specialized pharmacy-based clinics. 4. Develop, implement and establish new specialized pharmacy-based clinics as needed to provide clinical services for the service population. 5. Develops and implements clinical services quality assurance activities using predetermined established AAAHC, JC, and CMS standards. Works with established performance improvement (PI) committee groups to establish, implement and coordinate service unit wide PI programs. Participates in pharmacy PI activities using predetermined, established audit criteria. Works with the Quality Management coordinators in the resolution of identified problem areas. 6. Responsible, in conjunction with the Director of Pharmacy Services, for managing costs affecting the specialized pharmacy-based clinics at the designated branch of Pharmacy Services. 7. All other duties as assigned by the Director of Pharmacy Services. Education (Pharmacist I/II & Advanced Practice I/II): 1. Maintains and/or obtains any training, certification, or CE required for participation in specialized pharmacy-based clinics. 2. Develops hospital staff's pharmacological knowledge by participating in clinical programs; training pharmacy staff, students, interns, externs, residents, and health care professionals. 3. Counsels patients, their representatives, and patient family members on safe and effective use of, including compliance with, prescribed medications. Performance Improvement (Pharmacist I/II & Advanced Practice I/II): 1. Identifies and reports any medication errors or adverse drug reactions as per the Medication Error and Adverse Drug Reaction reporting policies. Identifies areas in TCRHCC pharmacy or hospital practice that could be improved so as to minimize patient safety risks and provide more efficient operations. 2. Selects, procures, prepares and dispenses only those medications that conform to standards of quality for potency, purity, and effectiveness at the time they are administered to or used by the patients. Evaluates all prescriptions for appropriateness to the age, weight and gender of patient. 3. Packages, manufacturers, and compounds products, including unit doses and intravenous additives (including chemotherapy), that are not commercially available as a premix. 4. Maintains necessary legal records on controlled substance drugs and other prescription products. 5. Serves on various hospital-wide committees as a representative of the pharmacy department. Implements the drug Utilization Review Reports and presents the review to the quality assurance committee through the pharmacy and therapeutic committee as needed. 6. Provides pharmacological information by answering questions and requests of health care professionals. Consults with physicians on the significance of properties of drugs regarding absorption, metabolism and excretion in all age groups. 7. Maintains pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
Qualifications: Education (Pharmacist I/II & Advanced Practice I/II): Doctor of Pharmacy or a Bachelor of Pharmacy degree from a school recognized by the American Council of Pharmaceutical Education Certification (Pharmacist I/II & Advanced Practice I/II): • Must have a current valid and unrestricted license to practice pharmacy in a State or Territory of the United States, or the District of Columbia • Must have a current valid Basic Life Support (BLS) Certification Experience: Pharmacist I • N/A Pharmacist II • A minimum of one (1) year experience as a Registered Pharmacist Advanced Practice Pharmacist I • Prefer PGY-1 Residency Experience • A minimum of two (2) years experience as a Registered Pharmacist • A minimum of one (1) year experience as a Clinical Pharmacist Advanced Practice Pharmacist II • A minimum of five (5) years experience as a Registered Pharmacist • A minimum of two (2) years experience as a Clinical Pharmacist If the applicant is hired as Pharmacist I, he/she will have the opportunity to move to Pharmacist II and to Advanced Practice Pharmacist I or II when the criterion has been met. To be eligible for Advanced Practice Pharmacist I or II he/she will become credentialed to work in the specialized pharmacy–based clinics in addition to the experience qualifications. Pharmacy experience criterion and the approval of the Director of Pharmacy Services will determine which tier the applicant will hold within this position.
