Sacred Peaks Health Center is an effort by the Tuba City Regional Health Care Corporation (TCRHCC) to provide more local care to 7,000 beneficiaries who live in the Flagstaff community. Sacred Peaks Health Center's mission is to heal, to respect, and to console, while providing quality and compassionate health care. Provides comprehensive and continuing health and medical care services in full scope Primary Care practice. The health care provided requires the recognition of diseases and injuries, which require referral including those which are difficult to discover or identify. This position will provide outpatient primary care services in the TCRHCC Flagstaff Clinic. |
Duties: | Within the Primary Care specialty area, assumes responsibility for the diagnosis, prevention, therapy, maintenance, and rehabilitation of patients in the capacity of a senior attending or expert. Typical, but not all-inclusive, assignments include the following: 1. Interview and examines patients, reviews past medical history, and requests and/or performs diagnostic tests and examinations deemed necessary to obtain all possible information related to each case. Makes preliminary diagnosis, directs, prescribes or provides treatment, or arranges for specialized care or patient referral as required. Bases decisions upon information obtained, professional medical knowledge and skills, and prescribed policies and procedures. Make notes of observations to be incorporated into clinical records. Provides emergency care as required. Provides definitive management in all medical categories for cases that do not require referral. Gives total patient care including prevention, health maintenance, early diagnosis, treatment and follow-up services to services to patients under his/her care. 2. Conducts outpatient clinics making observation appropriate to the outpatient; underlying disease and treatment plan, including current symptoms. Reviews patient's medications, diet, therapy course, adherence to management plan and patient observations. Modifies regimen as indicated. Instructs and counsels patients and family members as required. Performs or assists other physicians in the performance of minor surgery. 3. Refers patients to appropriate contracted medical or other government facilities, providing full clinical information for care and diagnostic procedures that cannot be adequately provided here. Coordinates and integrates information obtained into the ongoing Health Care Program at TCRHCC and assures the provision of timely follow-up care as required. 4. Assures preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Dictates clinical reports and correspondence concerning patients under his/her care to physicians, hospitals, and other individuals and facilities. 5. Works closely with allied health professionals (i.e. Community Health Nurse, Social Workers, Health Educator, etc.) to utilize and coordinate the services of other health care professionals in the management of the patient's medical, psychological and social problems. 6. Qualifications: | QUALIFICATIONS NECESSARY QUALIFICATIONS Education: A Medical Degree (MD or DO) from an accredited school of medicine. License: Must possess a valid, current, full and unrestricted license to practice as a physician in the state of Arizona. Credentialing: Must be credentialed to serve on the Medical Staff (provide all related information as requested and needed for credentialing process). Experience: Completion of three years of accredited residency training in Family Practice or Internal Medicine. Must be Board Eligible or Board Certified by the American Board of Family Practice or Internal Medicine. Other Skills and Abilities: A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas: • Positive working relationships with others • Possession of high ethical standards and no history of complaints • Reliable and dependable; reports to work as scheduled without excessive absences. |
