Summary: POSITION OPEN UNTIL FILLED. SALARY DEPENDS ON EDUCATION LEVEL, EXPERIENCE, AND CREDENTIALS. We operate a medical clinic located on the Hoopa Valley Indian Reservation, partially funded by the Indian Health Service. The clinic is solely owned and operated by the Hoopa Valley Tribe. We are a Joint Commission accredited ambulatory clinic and behavioral health unit, with our Patient Centered Medical Home accreditation. Our primary programs and services are outpatient medical, dental, pharmacy, outreach services, senior nutrition, ambulance service, a grant funded diabetes department, and behavioral health unit [which includes mental health and substance abuse]. Hoopa is located on the beautiful Trinity River in Northern California. Hoopa is a small community of approximately 3,000 residents primarily Native American. The position is a full-time, regular staff position that includes benefits such as: a 401[k] Safe Harbor Plan [100 vested, KMC up to 4 match] 12 paid holidays medical, dental, vision, and life insurance vacation leave of 8 hours per month, increases based on years of service and, sick leave of 8 hours per month [static]. Hours of work are Monday through Friday, 8AM to 5PM. Work weeks are based on 80-hour pay periods, and employees are paid every other Friday.

Diabetes Nurse Educator/Certified Diabetic Educator. FT, Regular. The Diabetes Program Manager/Certified Diabetes Educator will coordinate the Diabetes Management and Prevention Program and serve as the lead educator/case manager for the K'ima:w Medical Center [KMC] SDPI Competitive Grants Program and other available diabetes grants and projects. This will include developing, implementing, and evaluating a diabetes prevention lifestyle curriculum, participating in the development and implementation of related classes, demonstrations, and community and family-based activities and coordinate project activities and provide program management. In addition this position will provide outpatient education and case management to persons with diabetes who receive their care at KMC, and ensure that all programmatic activities are carried out in accordance with grant guidelines, project protocols, standards of care, and policies and procedures of KMC.
Duties: Functions and Responsibilities: In collaboration with the local diabetes project team, as well as, with the CGP Coordinating Center, IHS, other grantees, other health professionals, and community partner, the Diabetes Program Manager/Certified Diabetes Educator will: Diabetes Program Manager: 1. Oversee the biannual community assessments to establish community needs and facilitate prevention activities and upon completion of community assessments identify, coordinate and facilitate prevention activities. 2. Develop, collect and provide diabetes educational materials. 3. Oversee data to collect, establish method for obtaining and tracking data, and generate statistical reports to monitor and improve overall diabetes program. 4. Collect and analyze data and information for program planning and reporting 5. Monitor and maintain the diabetes grants goals, objectives, and budget. 6. Provide quarterly reports. 7. Prepare progress and data reports for submission to tribal review, IHS, and the Coordinating Center. 8. Supervise activities of Diabetes program staff. 9. Participate in the conceptualization, design, and development of all project protocols, procedures, educational and data collection activities, and infrastructure systems for successful implementation and evaluation of the project according to grant guidelines, tribal requirements, and local cultural values. 10. Participate actively in all national, regional, and local Grantee project development and training meetings. 11. Conduct annual Diabetes Audit as well as quarterly Audit reviews. 12. Prepare and submit grants for continued funding of program. Certified Diabetes Educator: 1. Clinically assess, identify, coordinate, educate, and case manage diabetes patients while working cooperatively with Diabetes Team, medical and nursing staff, and other members of the healthcare environment 2. Provide diabetes and nutrition training and education to individual participants, family members, community groups, and other project staff on all aspects of diabetes care as needed or required by the project protocols. 3. Conduct CVD risk assessments and provide appropriate case management and referral to all persons with diabetes. The focus will be on reduction of cardiovascular disease risk factors including obesity, hypertension, hyperlipidemia, etc. 4. Collaborates with the diabetes team to develop and implement a patient care plan that meets the IHS Diabetes Standards of Care. Evaluates clinical and educational outcomes and makes appropriate referrals 5. Oversee case manager in planning and coordinating monthly Diabetes Clinics. 6. Plan, implement, and maintain IHS Integrated Diabetes Education and Clinical Standards Recognition Program 7. Participate in the modification and development of the lifestyle balance curriculum and educational materials with other grantees, local project team members, and appropriate local community members to incorporate collaborative decisions on curriculum, eligibility criteria, evaluation measures, and local cultural values. 8. Implement the curriculum, conducting sessions or coordinating and monitoring other staff in conducting sessions according to the agreed upon curriculum, project protocols, and local cultural values. 9. Use a variety of teaching strategies, approaches, and methods that incorporate theories and concepts of adult learning, readiness for change, empowerment and motivational interviewing, and local cultural values. 10. Administer class evaluations, monitor data for areas needing improvement, and implement performance improvement activities. 11. Duties and responsibilities will include other activities and areas as needed to carry out the position functions
Qualifications: Minimum Education/Experience: • Bachelor of Science [BS] degree • Minimum five years experience providing diabetes education and program management • Experience direct patient care through care and education for patients with diabetes License/Certification: • Certified Diabetes Educator [CDE] or ability to obtain within one year. • Current California Registered Nurse license preferred. Other professional licenses or certifications with a CDE considered on a case-by-case basis. • Valid California Drivers License [CDL]
