POSITION IS OPEN UNTIL FILLED. SALARY RANGE DEPENDS ON EDUCATION, EXPERIENCE, AND CREDENTIALS. We operate a medical clinic located on the Hoopa Valley Indian Reservation, partially funded by the Indian Health Service. The clinic is solely owned and operated by the Hoopa Valley Tribe. We are a Joint Commission accredited ambulatory clinic and behavioral health unit, with our Patient Centered Medical Home accreditation. Our primary programs and services are outpatient medical, dental, pharmacy, outreach services, senior nutrition, ambulance service, a grant funded diabetes department, and behavioral health unit [which includes mental health and substance abuse]. Hoopa is located on the beautiful Trinity River in Northern California. Hoopa is a small community of approximately 3,000 residents primarily Native American. The position is a full-time, regular staff position that includes benefits such as: a 401[k] Safe Harbor Plan [100 vested, KMC up to 4 match] 12 paid holidays medical, dental, vision, and life insurance vacation leave of 8 hours per month, increases based on years of service and, sick leave of 8 hours per month [static]. Hours of work are Monday through Friday, 8AM to 5PM. Work weeks are based on 80-hour pay periods, and employees are paid every other Friday. Family Nurse Practitioner. FT, contract employee. The Family Nurse Practitioner will function as a primary care provider seeing all ages of patients and all medical conditions. Referral of complex medical conditions to Physician providers will be expected. The Family Nurse Practitioner will be expected to work in outpatient and urgent care settings as necessary. |
Duties: | FUNCTIONS RESPONSIBILITIES: 1. Must comply with all California State requirements including those applying to Title 22, and the Joint Commission for Accreditation of Health Care Organizations. 2. Incumbent will be expected to maintain complete and appropriate medical records and documentation on all services that they deliver. 3. Will be proficient at EKG interpretation, IV line insertion, oxygen therapy, PALS, and ACLS. 4. Must comply with the Kima:w Medical Center Medical Staff Bylaws. 5. The Family Nurse Practitioner will be expected to participate in all Medical Staff Meetings, Pharmacy and Therapeutics Committee meetings, and Quality Assurance Committees as requested. 6. Participates in peer review activities as necessary. 7. Appropriately manages primary care pediatric, adult, and elder health care. And appropriately refers when necessary. 8. Coordinates other specialty services for patients as needed. |
Qualifications: | EDUCATION and/or EXPERIENCE: Must be a Masters Degree Family Nurse Practitioner. CERTIFICATES, LICENSES, REGISTRATIONS: • Must have a current unrestricted California Family Nurse Practitioner License. • Must have current ACLS Certification. • Must have current PALS Certification. • Must have a Furnishing Number within three months of hire for prescription processing. |
