Phoenix Indian Medical Center Medical Records

The Phoenix Indian Medical Center Health Information Management (HIM) department is responsible for keeping complete medical records for each patient. Your original medical records will be kept with PIMC, but the information in it belongs to you. You have a right to access your health information.

Requesting Medical Records

To receive your medical records, you must complete the authorization form:

Upon receipt of your request, we may take up to 30 business days to process your medical record. Records will be released and delivered in the method indicated on the authorization form. The completed form along with a valid copy of your photo ID can be faxed, mailed, or dropped off at the medical records front desk. If you are requesting medical records for a child, please submit a copy of the child's birth certificate or legal guardianship paperwork along with your request.

Status Check

If you have not received your medical record within ten business days, please contact our Health Information Management (HIM) Department using one of the methods noted below.

No problem! We've made it easy for you. Simply use the convenient drop box located outside our ROI office to leave your medical records request, and our staff will follow up with you as quickly as possible.
