The Phoenix Indian Medicine Center (PIMC) offers patients a comprehensive Primary Care Medical Home (PCMH). What this means is that your care is provided through a team-based health care delivery model led by a health care provider to provide comprehensive and continuous medical care to obtain the best possible health outcomes.

While PCMC is not a walk-in clinic, we do have a limited number of same day appointments. These appointments are reviewed and scheduled based on medical need, by our triage nurse.

Records Requests and Submissions

To securely fax patient records, please send them to 602-263-1665 with your organizations cover sheet including any necessary patient information. If you are experiencing any issues or need confirmation of receipt, please call us at 602-263-1501.

The Clinic is the large, single story building on the North East corner of the PIMC Campus. Dedicated patient parking is available in front of the clinic entrance.
