Medical Records
The Phoenix Indian Medical Center Health Information Management (HIM) department is responsible for keeping complete medical records for each patient. Your original medical records will be kept with PIMC, but the information in it belongs to you. You have a right to access your health information.
Requesting Medical Records
To receive your medical records, you must complete the Authorization for Use or Disclosure of Protected Health Information Form [PDF 697 KB]
How to Submit Your Request
You may submit your completed authorization form and any required documentation using one of the options below.

In Person
Deliver your completed authorization form, photo ID, and any required documents to our ROI office during normal business hours.
After-Hours Drop Box
Requests may be placed in the secure drop box located outside our ROI office. Our staff will follow up with you as quickly as possible.

Processing Time
Upon receipt of your request, we may take up to 30 business days to process your medical record. Records will be released and delivered in the method indicated on the authorization form.
Check the Status of Your Request
If you have not received your medical records within 10 business days, please contact our Health Information Management (HIM) Department to check the status of your request.

Monday through Friday: 8:00 a.m. to 4:30 p.m.
Holidays observed by the Indian Health Service may affect processing times.
