Patient Registration

At Phoenix Indian Medical Center (PIMC), we welcome new patients and are committed to making the registration process as smooth as possible. Registering as a patient ensures we can provide you with the best possible care and properly coordinate your healthcare services.
What to Bring
When you come to register, please bring the following documents:
- Proof of Tribal Membership or American Indian/Alaska Native Descent – This may include a Tribal Enrollment Card, Certificate of Degree of Indian Blood (CDIB), or documentation showing that you are a descendant of an enrolled member of a federally recognized Tribe. You do not have to be an enrolled Tribal member to be eligible for IHS direct care services. Registration staff can help determine what documentation is needed to verify eligibility.
- Government-issued Photo Identification – such as a driver's license, state ID, or Tribal ID.
- Health Insurance Information – including Medicare, Medicaid, or private insurance cards, if applicable.
- Contact Information – including your current address, phone number, and emergency contact.
Bringing complete information helps us serve you quickly and ensures your medical record is accurate.
Registration Locations on Campus
You may register as a new patient at the following locations on the Phoenix Indian Medical Center campus:
- Patient Registration Office – Located near the Emergency Department, at the North entrance of the hospital.
- Specialty Clinics Registration Desks – Available for patients who are referred directly to specialty services.
If you are unsure where to go, our staff and volunteers at the Information Desk in the main lobby will be glad to assist you.

Emergency Care Policy
Emergency care is available regardless of eligibility for IHS services. Under the Emergency Medical Treatment and Labor Act (EMTALA), individuals who come to the Emergency Department requesting examination or treatment will receive an appropriate medical screening examination. If an emergency medical condition is identified, stabilizing treatment will be provided or an appropriate transfer arranged, as required by law.
Eligibility for IHS services may be verified after emergency care is provided. Patients who are determined not to be eligible for IHS services may be billed for services received.
