DE0191: Diabetes Online CDE: Role of Pharmacy Clinicians in Hypertension, Diabetes, and Lipid Management
This session will provide discussion of strategies for using evidence-based interventions and a team approach to help adults with Type 2 diabetes, hypertension and lipids. Common challenges with addressing patient issues will be presented and discussed. The importance of individualized assessment and treatment strategies will be emphasized. The process of developing an individualized and multidisciplinary plan of care focusing on the patient’s physical, psychosocial, spiritual, emotional, and cultural characteristics/beliefs will be addressed.
1. Collaborate with patients to develop an individualized plan of care focusing on the patient’s physical, psychosocial, spiritual, emotional, and cultural characteristics/beliefs.
2. Assess and modify, if necessary, a medication regimen to control type 2 diabetes, lipids and hypertension.
3. Describe the importance of setting realistic treatment goals that aim to reduce or address health disparities and barriers to patient care.

Tuition must be paid in full 8 weeks prior to the start date of any course. Request for refunds must be received in writing at least two weeks before the course begins. For each refund request, there will be an administrative charge of $100. No refunds will be made to registrants who fail to attend a course. If IHS CDE program cancels a course, then 100% of the tuition will be refunded.
