Uniform Data System
The Uniform Data System (UDS) is a standardized set of data reported by federally funded programs such as the Indian Health Service (IHS) Urban Indian Organizations (UIOs), UIOs funded by the IHS are required to produce annual UDS reports.
The UDS standard set of annual reports include aggregate information from various UIOs regardless of the electronic health record (EHR) system utilized. These reports provide an overview of patients and visits at a UIO, including aggregated total number, age, gender, zip code of residence, insurance sources, race/ethnicity of patients, number of visits by provider type, by key diagnoses and services, characteristics of special populations, quality of care indicators, health outcomes, and disparities.
The UDS National Summary report is an annual calendar year report. UIOs must report on the entire calendar year. This is true even for UIOs whose designation or funding begins, either in whole or in part, after January 1 or ends before December 31. Similarly, UIOs with a fiscal year or grant period other than January 1 to December 31 will still report on the calendar year, not on their fiscal or grant year.
The UDS National Summary report does not contain any personally identifiable information (PII) data that could potentially identify a specific individual.

In addition to the UDS, the Budget Formulation (BF) report is a standardized set of data reported by the UIOs. The BF report is generated annually and submitted to the National Council of Urban Indian Health (NCUIH) to present at the National IHS Budget Formulation meeting. The report provides an annual set of standardized metrics that provide details, including aggregated total visits, aggregated total patients, visits/patients by Area, visit type, visits/patients by State, patient and visit count details and UIO contributor information.
The report definition is based on the calendar year, unduplicated number of clients, and total number of encounters (Ambulatory, Telecommunications, and Chart Reviews) by Tribal Membership. The report communicates to stakeholders the number of clients and encounters being provided by the UIOs to members of a specific tribe.
