• Audience: IHS Employees, Organizations (Tribes, Governments, Private Sector).
• Purpose: The https://www.ihs.gov/suicideprevention/communityguidelines/ provides guidance to tribes and IHS facilities on how to respond to a suicide related community crisis. I would like to add a form where tribes or IHS facilities can request assistance as now there is no corridor to request such assistance. The form would need to be fillable with limited space in filled fields to protect PII.
