What is in the PATH Patient Portal?

The PATH Patient Portal is a secure online tool for patients who qualify for health care under
the Indian Health Service (IHS) and visit sites who have migrated to PATH EHR.

Within the PATH Patient Portal, you will have access to:

• View medication list and request refills
• Test results
• Clinical documents
• Visit summaries view, which is download and transmittable
• Secure messaging with your provider and the care team
• View, schedule, and request appointments
• View allergies
• View list of immunizations

How do I access my account?

Patients who qualify for health care under the Indian Health Service and their authorized
representatives can register to use the PATH Patient Portal.You will need to set up your username,
password, and verify your identity using ID.me, a secure identity verification service to sign in to the
PATH Patient Portal.
