If your child requires urgent care during weekdays, please call the clinic to speak to the Pediatric triage nurse by calling 602-263-1507 and they will make sure you get the most specialized care needed. If your child’s condition is emergent, the PIMC Emergency Department is there for them 24/7. If they have a critical/life threatening condition, call 911.
Our PIMC Pediatric Clinic staff are dedicated to providing your children with the highest quality pediatric care in the most professional, safe, culturally sensitive, and caring environment.
Our Providers specialize in preventative, chronic and acute medical care for children from birth to 21 years of age. When your child “ages out” of pediatric care, we will seamlessly transition them to the PIMC Primary Care Medicine Clinic (PCMC) for services. We offer after hours phone access to help assist you with any urgent questions you may have concerning the health of your child.
New Patients
To help make registration as easy as possible, please bring the following information and documents:
- Proof of Tribal Membership or American Indian/Alaska Native Descent – This may include a Tribal Enrollment Card, Certificate of Degree of Indian Blood (CDIB), or documentation showing that the child is a descendant of an enrolled member of a federally recognized Tribe. A child does not have to be an enrolled Tribal member to be eligible for IHS direct care services. Registration staff can help determine what documentation is needed to verify eligibility.
- Government-Issued Photo Identification – A driver's license, state-issued ID, Tribal ID, or other government-issued photo identification for the parent or legal guardian.
- Health Insurance Information – Medicare, Medicaid, or private health insurance cards, if applicable.
- Contact Information – Current address, phone number, and emergency contact information.
When Someone Other Than a Parent Is Caring for the Child
If a child is in the legal custody or care of someone other than a parent, documentation may be required to show that the individual has authority to consent to medical treatment for the child. Please bring the applicable documentation:
- Guardianship Order – A legal document issued by a court or other authorized tribunal that gives a designated guardian the legal authority to make decisions on behalf of the child, including medical decisions as specified in the order.
- Consent by Proxy [PDF – 136 KB] – Documentation authorizing another person to consent to medical treatment for a minor on behalf of the child's parent or legal guardian.
Medical Records Requests and Submissions
To request copies of your child’s medical records, submit medical records, or authorize the release of health information, please visit our Medical Records page for information and instructions on how to complete these requests.
