Our PIMC Pediatric Clinic staff are dedicated to providing your children with the highest quality pediatric care in the most professional, safe, culturally sensitive, and caring environment.

Our Providers specialize in preventative, chronic and acute medical care for children from birth to 21 years of age. When your child “ages out” of pediatric care, we will seamlessly transition them to the PIMC Primary Care Medicine Clinic (PCMC) for services. We offer after hours phone access to help assist you with any urgent questions you may have concerning the health of your child.

The Clinic is the large blue-gray building located towards the back of the campus. Dedicated patient parking is available in front of the clinic entrance.

Schedule an Appointment

To schedule, change, or cancel an appointment with the Pediatric Clinic, please call (602) 263-1507 from 6am to 6pm. If your child requires urgent care during weekdays, please call the clinic to speak to the Pediatric triage nurse and they will make sure you get the most specialized care needed. If your child’s condition is emergent, the PIMC Emergency Department is there for them. If they have a critical/life threatening condition, call 911.

Please note that all patients under 18 years brought to the clinic without a parent or guardian will require a notarized Proxy Form (see below). Parents may sign the Proxy Form in person at any visit without needing notarization. The form is valid for one year.

Certificate of Indian Blood/Tribal Certificate/Tribal ID**
Birth Certificate**
Social Security Card**
Guardianship order
Consent by Proxy [PDF - 136 KB]
Delegated authority to a non-parent/guardian the legal right to consent to medical treatment for or a minor.

**These documents are required for the eligibility of the child to be established at PIMC.
