National CHR Conference
This is CHR: Our Story, Our Relationships, Our Responsibility. Defining Our Practice. Demonstrating Our Impact.
Community Health Representatives (CHRs) have long served as trusted connections among individuals, families, Tribal communities, health care systems, and community resources. The National CHR Conference is designed to honor that foundation while strengthening the knowledge, skills, relationships, and systems needed for CHRs and CHR programs to thrive in a changing health care environment.
The conference will bring together CHRs, supervisors, Tribal leaders, health care partners, program staff, and others committed to strengthening the CHR workforce. Participants will have opportunities to build practical skills, learn from other CHR programs, strengthen partnerships, share promising practices, and explore approaches that support high-quality, culturally grounded, community-based care.

CALL FOR POSTERS: Submit by December 4th
Fill out the form and upload your poster content on the conference call for posters page
Purpose of the Call
The Call for CHR Program Posters invites CHR Programs from across American Indian and Alaska Native communities to share stories, highlight the relationships that support their work, and showcase the important role they play in serving their communities. The poster session will provide an opportunity for programs to connect, learn from one another, and celebrate the impact of CHR work. Rather than a traditional academic research poster session, this will be a national program showcase focused on sharing experiences, successes, and lessons learned.
Poster Story Framework
All selected posters will follow a shared content framework to create a cohesive national showcase, while offering 2–3 approved visual layout options to give each CHR Program an opportunity to reflect its own story and personality. This approach will provide consistency across the collection while ensuring that each poster feels unique and engaging.
Fifty CHR Program posters will be selected to highlight the contributions and future priorities of the CHR workforce across American Indian and Alaska Native communities. Each program will provide the content that tells its story, while the conference contractor will transform the submissions into professionally designed posters and coordinate final printing.
