Alzheimer's Infographic

The Elder Health and Alzheimer's Programs use a tribally driven, coordinated approach to improve dementia care.

This infographic shows how the program partners with tribal communities to support timely diagnosis, expand screening, strengthen the workforce through training and events, and share information through newsletters, social media, and outreach activities. It also highlights how the program uses data to guide improvements and support clinical care in ways that honor culture, community priorities, and local strengths.
